Registration Authority

IEEE offers Registration Authority programs or registries which maintain lists of unique identifiers under standards and issue unique identifiers to those wishing to register them. The IEEE Registration Authority assigns unambiguous names to objects in a way which makes the assignment available to interested parties.

IEEE 1451.4 MANUFACTURER ID APPLICATION

Requestor/company hereby agrees to indemnify and hold IEEE harmless from any claim arising out of Requestor's use of assignments or misuse of any assigned number. Signor agrees to notify IEEE of any changes of address or contact information. IEEE Registration Authority is unable to process any application without acceptance of the indemnification statement.

This form may be used to request a manufacturer ID and can be faxed or mailed. Please submit a separate form for each request. IEEE does not honor requests for applicant-specified identifiers.

Requests for a manufacturer ID will be honored within 7 business days of receipt of the fee. Requests for a second manufacturer ID, by an existing holder, must be accompanied by an affidavit on company letterhead, confirming that the new number will not be used until the usage of the original number has reached 95%.
