ACPCTREEID INFORMATION CHANGE FORM
Complete this form if there have been one or more of the following changes since the AcpcTreeId assignment was issued to your company.
- If there is a company name change due to purchase and/or sale.
- Email the IEEE-RA a copy of a press release or some details of the company name change.
- If your company has more than one of each assignment that must be changed.
- Complete a separate form for each. (Example, if a company has two PSID assignments, complete two forms so that the contact information can be updated for both of assignments.)
