NOMINATION FORM
IEEE-SA International Award


The award may be presented annually to an IEEE-SA individual member (who is also an IEEE member) who has made extraordinary contributions to the international goals of the IEEE-SA.

1. Name of Individual.

2. Address (include email).

3. State succinctly why this individual should be so honored. (It is suggested that this summary should be written after the nomination is prepared).

4. Proposed citation (no more than 25 words).

Please give as much information as is possible for each of the criteria listed under points 5-8 for the "IEEE-SA International Award."

The nomination will be judged on the information you provide along with the supporting statements by the references.

5. Describe the standards related contributions, other than participation on standards writing committees, that the nominee has made to advance the international goals of the IEEE-SA.

6. Describe how these standards related contributions have precipitate changes in the global marketplace, and the significance of those changes.

7. Describe how the nominee's initiatives, directly or indirectly, have served to enhance the acceptance of IEEE Standards internationally.

8. Please provide any additional information that may not have been covered in the above points, but that you feel will reinforce this nomination.

9. Endorsers (names, addresses, and email). Nominators are advised to seek endorsers who are familiar with the candidate's roll in activities supporting achievements of SA international goals. Endorsers shall be advised to include in their support letters a statement indicating how long they have known the candidate and in what capacity. A minimum of three supporting letters required; maximum of five permitted. No more than five letters will be reviewed by the Committee. Letters may be sent electronically via e-mail to the IEEE-SA Awards Committee.
