This form starts the Sponsor Ballot process and should only be filled out by IEEE Standards Sponsor Chair, Standards Liaison, Standards Coordinator or Working Group Chair.

- If your initial invitation to ballot began before March 7, 2005 (using the old balloting system) you must submit this form to begin the ballot process.
- If your initial invitation to ballot was conducted on or after March 7, 2005 using myBallot™, you must log onto myBallot to conduct ballot.

IEEE Standards Sponsor Ballots are the means by which consensus is achieved on a proposed IEEE Standard. A ballot is usually conducted for 30 days, however this period is flexible. If you have not yet created your ballot group through the invitation process, please submit the Invitation Request Form.

Please fill out all fields (those fields with an * are required) in this form to begin processing your IEEE Standards Sponsor Ballot. When you click "submit," this information will be sent to the staff liaison you identify on this form. It normally takes 3-5 business days to process a ballot request.
