IEEE copyright permission letters have been modified (2009) and are shown in Annex D of the 2009 IEEE Standards Style Manual. Copyright permission request letters and response letters are given for material excerpted without change and for material that is excerpted and modified. The copyright permission request letters and response letters for both types are available as form letters.
