If there is a company name change due to purchase and/or sale please also fax a press release or some details of the company name change to +1 732-562-1571 or e-mail the web page link to the IEEE Registration Authority .
If your company has more than one of each assignment that must be changed, please complete a separate form for each. For example, if your company has two OUI assignments, please complete two forms so that the contact information can be updated for both of the assignments.
